Dental decay is the most common chronic disease in children. Fluoride varnish (FV) is a preventive oral health service with proven effectiveness at reducing dental caries in dental and primary care settings. The objective of this study was to determine how long it takes to apply FV treatments during primary care well visits to address one of the most common barriers as reported by pediatricians – lack of time. FV treatment videos were collected at six clinics in Georgia with rigorous time studies conducted on each video to determine the Standard Time for the FV treatment process as well as the FV Application Component of the process and reasons for delays. Median Standard Times varied by clinic, ranging from 67.7 seconds to 166.9 seconds with an overall median of 109.7 seconds. This results in per FV application labor costs of approximately $2.38 for pediatricians, $1.16 for registered nurses, and $0.53 for medical assistants. Findings from this study support the inclusion of FV applications as a common practice during primary care well visits.

In a lean production environment, reduced setup times can lead to many benefits, including reduced lead times. Previous research has primarily relied on the SMED methodology and mathematical modeling to reduce setup times at machine centers – and both are very useful techniques. We use the Soft Systems Methodology, combined with the Seven Tools of Quality, to provide a structured, illustrative means for diagnosing production and quality issues. A baseline average setup time was established by which future setup times would be compared. The intervention included brainstorming sessions between management and the converting center work crew that disclosed many reasons for increased setup times, some of which were under management’s control. Our findings resulted in a 24% reduction in average setup times and a 62% reduction in the moving range at a bottleneck machine center in the corrugated box industry.

Root cause analysis techniques are often applied to problems in the workplace; however, they may also prove very useful to home projects. This research explores the application of two root cause analysis techniques in home projects: (1) 5 Whys to determine the root cause of a home air conditioning unit that runs continuously but does not cool, and (2) an innovative Lean PFMEA to repair a John Deere riding mower that starts, then stops. Employing the 5 Whys technique led to the discovery of incorrect color-coded wiring from the original air conditioning unit to the thermostat. Lean PFMEA enabled a correct diagnosis and resolution of the mower start/stop issue via a Kaizen event, grass clippings in the fuel line, which was remedied by cleaning the fuel tank and replacing the fuel lines, fuel filter, and carburetor. These techniques provide Lean methodological approaches to problem-solving, which often leads to reduced homeowner aggravation, repair time, and repair expense.

The Single-Minute Exchange of Dies (SMED) methodology proved to be an effective approach for reducing setup times on a bottleneck 66” Koppers rotary die-cutter machine center in a corrugated box plant. Root cause analysis techniques, such as a check sheet and control charts, aided the analysis. The supervisor and work crew, consisting of both the operator and setup person, played an integral role in reducing setup times by consenting to being videotaped during a typical setup and then breaking down the video to distinguish internal versus external steps. By converting many internal steps to external steps, average setup times were reduced from 55 minutes to 32 minutes, a 42% improvement.

Root cause analysis techniques are often applied to problems in the workplace; however, they may also prove very useful when applied to home projects. This research explores the application of root cause analysis techniques in three home projects: (1) Cause-and-Effect diagram to remove Palmetto bugs in a condo dwelling, (2) Five Whys method to repair a sunroof water leak in a car, and (3) Fault Tree Analysis to repair a Toro string trimmer that starts, then dies. The effective use of root cause analysis techniques can have a meaningful impact on resolving home project issues resulting in a restoration of homeowner issues as well as reduced homeowner anxiety, repair time, and repair cost.
